Background & Purpose
As an Accountable Community of Health (ACH), BHT has been facilitating care coordination in the region since our founding. From our early days of insurance navigation and trusted messenger work, we have prioritized investing in and supporting the community based workforce and their supporting organizations who provide access to care and vital services to so many in our region.
Today, we have celebrated 12 years as an organization and invested almost $90 million into health delivery systems and coordinated social care. We are looking forward to what is next for our region, and have been working alongside our other ACH partners throughout the state to develop ourselves as a Community Care Hub, connecting folks to the support and social services they need through a trusted and trained community based workforce.

Funding
BHT intends to fund the care coordination work of 10-20 Community Based Organizations employing 12-30 Community Based Workers between February 2026 and June 2027. Proposals should be for care coordination services provided by a minimum of one to a maximum of five (1-5 FTE) Community Based Workers (CBW) and a recommended .3 Supervisor FTE per CBW.
is expected to be $234,000 for placement of one CBW at one organization This amount can cover staff salary and benefits, administration, relevant and appropriate technology costs, related travel, and client incidental costs.
Any contract awarded as a result of this procurement is contingent upon funding availability. BHT will negotiate contracts with payers (state, federal, local, managed care, etc.) for sustainable and ongoing funding to meet community needs and community health outcomes, creating a braided funding model. BHT will determine the best funding streams to meet partner and community needs, and the partner will bill BHT directly.

Who Should Apply?
Are you already providing community based care coordination in the BHT Region?
The following are the minimum qualifications for Bidders:
- Licensed to do business in the State of Washington / Holds a WA business license
- Must be a 501c3 under 509a1 status for Public Charities or local governmental agency (such as a local health jurisdiction or public hospital), or a Tribal Nation
- At least a year of experience providing community care coordination in alignment with BHT standards within BHT’s region (Adams, Ferry, Lincoln, Pend Orielle, Spokane, Stevens Counties and/or Colville Confederated, Kalispel, or Spokane Sovereign Nations)
- Physical presence in BHT service area (Adams, Ferry, Lincoln, Pend Orielle, Spokane, Stevens Counties and/or Colville Confederated, Kalispel, or Spokane Sovereign Nations)
- Agreement to use BHT’s Client Management System (CMS)
- No exclusions from sub-awards on Sam.gov
- Proof of Liability Insurance
- Capacity and commitment to providing Health Benefit Exchange insurance navigation services with trained staff
- If a previous contractual relationship is relevant, contract performance will be considered in the review process/decision criteria for selection

Note: BHT may modify these documents before contracting with awardees. We will let awardees know what, if any, revisions are made.
#1-3 are the signed components of a fully executed Community Care Hub contractual agreement between BHT and the awardee.
- Master Service Agreement (MSA) – The MSA outlines legal terms and conditions of the contractual agreement between the awardee and BHT.
- Statement of Work (SOW) – The SOW details the work of the awardee as a Community Care Hub partner. Includes the description of services, budget and payment, key performance indicators, Community Care Hub standards, quality improvement and support, and required training. The SOW sits under the terms of the MSA.
- Consent to Public Statements – Agreement by awardee and BHT to allow each other to make public statements about participation in the Community Care Hub and what information can be shared.
#4-7 are additional requirements as part of the Community Care Hub contractual agreement.
- Business Associate Agreement (BAA) – The BAA outlines the use, handling, and safeguarding of Protected Health Information (PHI) to meet HIPAA requirements. Signed alongside #1-3, unless partner already has a current BAA with BHT.
- Audit Risk Assessment – The assessment is documentation related to federal audit compliance. The results are used to identify areas of risk where mitigation strategies may be needed and documented. See Exhibit D of the SOW for details.
- Training Log and Attestation – An attestation form used for proof of completion for required training modules for community-based workers and supervisors covered by the SOW. See Exhibit H of the SOW for details.
- Navigator Cooperative Agreement – As part of being a Community Care Hub partner, awardee will also complete a Navigator Partner Cooperative Agreement and training with BHT to provide WA Health Benefit Exchange (WAHBE) Navigator Services for the purpose of enrolling qualified individuals in insurance. See Exhibit E of the SOW for details.

Through partnership with Better Health Together, community based organizations also learn the requirements of federal funding, equipping them to pursue additional funding streams at the highest level.

Information on the Request for Proposals (RFP) will be posted to the BHT website and sent out via our email list (make sure you are subscribed!). Open office hours for support and questions will begin on July 15th and run every Tuesday at 11am through the end of September.
