Dr. Eric Coleman is a leading national expert on case transitions, and will guide us thru some intensive, interactive local case studies (submit yours below). We will look at challenges and tools around communication, client self-management, and the business case for better care transitions.

Call for Case Studies
We're currently gathering local case studies for this training. We are looking for cases that illustrate transitions of care challenges around communication (internal or with external partners), paying for care transitions, and preparing patients for self-management. These can be transitions from residential, hospital, or inpatient settings to lower levels of care or into the home/community.

This is an opportunity for free technical assistance from a national expert on Transitions of Care. Don't miss it! If you have a case you would like us to look at, please email me with a brief description of your case. Please send your cases by Oct. 11.
