Background

Since BHT began over 12 years ago, we have been working to connect people to the right care, at the right time, from a provider that gets them! We envision a system that works for people, not a system that makes people work.  For the last year, we have been working to pilot a new system of care in our region called the Community Care Hub.

What is a Community Care Hub?

The Community Care Hub is a care model that connects folks to a worker at a local organization who can work with them step-by-step to find the right basic, medical, and cultural support for their unique needs.

So, where do community based organizations fit in?

A Community Care hub runs best when we are able to partner with local organizations that serve their communities. BHT is launching a request for proposals (RFP) for our latest cohort of Community Based Care Coordination partners, and you could be a part of it! We are looking for 10-20 organizations that already provide community based care coordination to join our network starting in 2026.

Funding

BHT intends to fund the community based care coordination work of 10-20 community based organizations employing 12-30 community based workers between February 2026 and January 2027. Proposals should be for care coordination services provided by a minimum of one to a maximum of five (1-5 FTE) community based workers (CBW) and a recommended .3 Supervisor FTE per CBW.

The amount awarded will be $165,148.05 per CBW in the proposal. This amount can cover staff salary and benefits, administration, relevant and appropriate technology costs, related travel, and client incidental costs.

Any contract awarded as a result of this procurement is contingent upon funding availability. BHT will negotiate contracts with payers (state, federal, local, managed care, etc.) for sustainable and ongoing funding to meet community needs and community health outcomes, creating a braided funding model. BHT will determine the best funding streams to meet partner and community needs, and the partner will bill BHT directly.
