Y3 contract information - August Cohort

This page has materials and information related to Year 3 Medicaid Transformation contracts for our primary care and behavioral health partners.

BHT will hold workshops with providers before final plan submissions are due. Following submission, the BHT team will review and approve the plan and place elements into contract. Contract will then be sent back to partners for final review and signature.

Year 3 contracts run December 1, 2021 – November 30, 2022.

One-on-One Workshops - October 4-8
BHT will host a 1-hour transformation plan workshop (virtual) with each partner. Partners will have a chance to discuss and receive feedback from BHT staff and consultants on their draft Transformation Plans. Sign up for your workshop time here:

Y3 Transformation Plan Template - draft due at workshop, final version due November 1.


Budget Template - please continue using the same Excel document from previous contract years, completing the “Summary Budget - Year 3” tab.

Y3 contract amounts:

  • Large volume: $284,900

  • Medium volume: $85,500

  • Small volume: $17,900
