This month’s meeting is required for BH/SUD providers in the August Cohort, and is open to any partners who are interested. REGISTRATION REQUIRED (including required attendees) - link below.

Breakfast & networking 8:30-9:00am. Lunch will also be provided.

Topic: Working with Clients with Serious Mental Illness to Improve Chronic Physical Health Outcomes

Training Purpose: A half day workshop for staff working with clients with serious mental illness to address their chronic medical conditions, connect with primary care, and tools to use in their work.  

Intended Audience:

– Case managers, peer specialists, patient navigators, care coordinators and medically trained staff (MA, LPN, RN, etc.)  working with clients with serious mental illness and coordinating with primary care

– Supervisors and managers championing the whole person care team and overseeing staff in new integrated care roles
